How to obtain Photo ID / Security Badge for accessing DLAM Facilities as an affiliate
(For Example: Visiting Investigators, Volunteer Employees or Temporary Laboratory Personnel)

ALL Steps Must Be Completed in Order

1. Download and complete the form located at the following link: Photo ID Application.

Follow the instructions on the form!!   As an affiliate, you will need to obtain a signature from your department's authorized signatory.  In order to avoid multiple trips to the Photo ID Office call: (310)-825-3258  to obtain contact information of the authorized signatory for your Investigator or Department.  The form also contains the location of the Photo ID Office.

NOTE: DLAM does not sign Photo ID Applications on behalf of Investigators or their associated Departments!!

2. Return the completed form , signed by an authorized signatory, to the Photo ID Office to obtain your Photo ID.

As an affiliate, you may also be required to establish a Bruin Online Network Login ID, depending upon your access needs.  (For example: If you will be logging in to UCLA Campus network computers as part of your duties.) Have your sponsor visit the following website for more details: BOL Guest Login Information

3. Complete all required certification coursework and/or DLAM sponsored training, as required, for working with animals approved on your Investigator's protocol.

4. Bring your newly obtained Photo ID to the DLAM Security Office for PIN number instructions and to establish the appropriate area access privileges.
